At Craft Behavioral Health we offer In-Office Medical Marijuana Assessments. 

If a patient does not qualify for treatment with medical cannabis during the initial assessment, they will not be charged for the appointment. 


Our Pricing:  

Initial visit- $100 (can not be combined with other discounts or promotions)

Recertification every 7 Months- $100  


FOLLOW-UP APPOINTMENT: Florida law requires that a patient meet with their provider two times per year. You will follow up with your provider in person or Virtually for reassessment  every 7 months. Your medical team at CMMC can be reached at any time to answer questions and assist you with your treatment. Contact your patient care coordinator or medical professional at (904) 518-6999.


PLEASE BE ADVISED THAT WE DO NOT PRESCRIBE SMOKEABLE MEDICAL MARIJUANA (FLOWER) INITIALLY. YOU MUST FAIL OR DEMONSTRATE A LIMITED RESPONSE FROM OTHER ROUTES OF ADMINISTRATION (ORAL, SUBLINGUAL, INHALATION, TOPICAL, ETC.). AFTER YOUR REQUEST IS VERIFIED DURING YOUR RENEWAL, WE CAN ADD SMOKEABLE MARIJUANA TO YOUR ORDER WHEN WE SUBMIT YOUR RENEWAL. YOU MUST COMPLETE A RENEWAL APPOINTMENT EVERY 210 DAYS (7 MONTHS) TO KEEP YOUR MEDICAL MARIJUANA ORDERS AND CARD ACTIVE. PLEASE NOTIFY CRAFT CLINC BY EMAIL IF YOU NO LONGER WANT TO STAY IN THE MEDICAL MARIJUANA USE REGISTRY. NOTIFICATIONS MUST BE RECEIVED AT LEAST 30 DAYS PRIOR TO THE CANCELLATION DATE.

Qualifying Conditions:

Florida MMJ is legal for medical use for patients with one of the following “Debilitating Medical Condition” which means: Cancer, Epilepsy, Glaucoma, Positive status for human immunodeficiency virus (HIV), Acquired immune deficiency syndrome (AIDS), Post-traumatic stress disorder (PTSD), Amyotrophic lateral sclerosis (ALS), Crohn’s disease, Parkinson’s disease, Multiple sclerosis, A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification, Chronic nonmalignant pain, Other debilitating medical conditions of the same kind or class as or comparable to those enumerated, and for which a physician believes that the medical use of marijuana would likely outweigh the potential health risks for a patient.
