New Patient Orientation

We are accepting new patients. Accept most commercial insurances and Self pay

We have numerous happy patients as evident from their positive reviews, and we hope you will have a good experience with us. In order to have enough time to complete all necessary paperwork, please arrive one hour early for your initial appointment if you haven't already printed and filled out the New Patient Packet as well as the New Patient: Adult ADHD Screener; if you are able to fill out the paperwork before arriving to the office for your first visit, please arrive twenty minutes before your appointment, as you will still need to complete and sign other documents. Please do not fax or e-mail the New Patient Packet , as your privacy cannot be guaranteed using such methods of communication.

If your child needs to be evaluated for ADHD, download and complete both the New Patient Packet and the New Patient: Child ADHD Screener. Additionally, please ask one of your child's teachers to fill out the ADHD Teacher Rating Scale before bringing your child in to be seen.

Please be sure to bring the following:

  • Insurance Card

  • Picture ID

  • List of medications currently taking

  • Custody or Guardianship papers, if applicable.

Co-pay is due at time of service. We accept cash, credit, debit, HSA, or FSA as long as they have a Visa or MC logo. We do not accept Amex or checks.

Please arrive 15 minutes early for subsequent appointments.

Please be aware that we will need half an hour to an hour to process your visit before you may see the provider, as we must undergo several steps to make sure that you are provided the best care.
