We are offering Telepsychiatry appointments to our established patients. Ist appointment is always in person. Afterward if you chose to have a telepsychiatry appointment you can do so.

Follow Up Form

Your form must be submitted on the day of your appointment before you can be seen for your appointment. The follow up form will be in your email either attached to your appointment reminder or in its own email or a link in text message. How to Get Started for Your First Virtual Appointment

  1. Download the Google Chat app on your phone

  2. Sign in to your Google chat with Gmail account

  3. *You can also go to Chat panel in your Gmail account on your computer, but your computer must have camera and microphone capabilities.*

  1. Inside your Chat App Click the green plus sign button at the bottom left.

  2. Enter info@nepajax.com and choose invite.

  3. Wait for us to accept your invitation, then send us a message containing the following information:

    1. Your name and date of birth

    2. Your appointment date and time

    3. Did you pay your copay? Did you pay through your online bank portal or want to use Credit Card on File?

    4. Did you complete your follow up form? Send your pharmacy information if not on file already: Name, Phone Number, zip code Your medication name/dosage and if you want to make any changes to your medication

  4. Respond to any and all messages you receive from us. When you receive the link, click it and press join. DO NOT LEAVE THIS SCREEN. Your provider will join the call shortly.

How to do Follow Up Virtual Appointments

  1. Open the Google Chat app on your cell phone (You can also open chat in your Gmail on your computer by clicking chat in your Gmail)

  2. If asked to, log in to your Gmail account

  3. On the home page, you should see our name: Craft Clinic. Click our name. This will open the chat.

  4. Send us a message containing the following information:

    1. Your name and date of birth

    2. Your appointment date and time

    3. Did you pay your copay? Did you pay through your online bank portal or want to use Credit Card on File?

    4. Did you complete your follow up form? Make sure to take vitals (height, weight, Blood pressure, pulse, respiratory rate, temperature) and put them in follow up form. If your you are using a new pharmacy or different pharmacy then please send the address of pharmacy.

Respond to any and all messages you receive from us. When you receive the link, click it and press join. DO NOT LEAVE THIS SCREEN. Provider will join the call.
